What is an Anomaly Scan?

An anomaly scan, also known as a Level 2 ultrasound or TIFFA scan (Targeted Imaging for Fetal Anomalies), is a detailed mid-pregnancy ultrasound performed between 18 to 22 weeks of pregnancy. It is one of the most important scans during pregnancy as it provides a comprehensive assessment of the baby's anatomy and development.

Unlike a routine pregnancy ultrasound, the anomaly scan involves a thorough examination of each organ system of the baby. The sonographer carefully evaluates the baby's brain, spine, heart, kidneys, limbs, face, and other structures to detect any structural abnormalities or birth defects. It also checks the placenta position, amniotic fluid level, and umbilical cord.

At Anubhav Life Care Barasat, we perform anomaly scans using our advanced Samsung V8 USG machine, which provides high-resolution images for accurate diagnosis. Our experienced radiologists ensure that every scan is thorough and reliable, giving expectant parents confidence and peace of mind.

What Does the Anomaly Scan Check?

The anomaly scan is a systematic examination of the baby's entire anatomy. Here is what our radiologists evaluate during the scan:

Brain — structure, ventricles, cerebellum
Spine — vertebral alignment, spinal cord
Heart — four chambers, major blood vessels
Kidneys — size, shape, urine output
Limbs — arms, legs, hands, feet, fingers, toes
Face — eyes, nose, lips (cleft lip screening)
Stomach — presence and size
Bladder — filling and emptying
Placenta — position and grading
Amniotic fluid — volume assessment
Umbilical cord — three-vessel cord check
Overall fetal growth — head, abdomen, femur length

When to Get an Anomaly Scan Done?

The ideal time for an anomaly scan is between 18 to 22 weeks of pregnancy (roughly 5 months). During this window, the baby is large enough for the organs to be clearly visible on ultrasound, yet small enough to fit within the scan field for a complete assessment.

Important Timing Note

If the scan is done too early (before 18 weeks), some structures may not be developed enough to assess. If done too late (after 23 weeks), the baby's position and size may make it harder to get clear views. Your gynaecologist will advise the best time based on your pregnancy dates.

How to Prepare for an Anomaly Scan

Full Bladder

Drink 2–3 glasses of water about 30 minutes before the scan. A moderately full bladder helps get clearer images, especially of the lower uterine segment.

Loose Clothing

Wear loose, comfortable clothing that allows easy access to your abdomen. A two-piece outfit is ideal so you don't need to change into a gown.

No Fasting Required

You can eat normally before the scan. There is no need to fast. However, avoid heavy meals right before the appointment.

Carry Previous Reports

Bring your earlier ultrasound reports (including the NT scan) and prescription from your gynaecologist for reference.

What to Expect During the Scan

The anomaly scan typically takes 30 to 45 minutes, depending on the baby's position and cooperation. Here is what happens during the procedure:

  1. 1You will be asked to lie on your back on the examination bed. The room will be dimly lit for better screen visibility.
  2. 2A warm, water-based gel is applied on your abdomen. This gel helps the ultrasound waves travel smoothly.
  3. 3The sonographer places a transducer (a smooth, handheld device) on your abdomen and gently moves it to capture images of the baby from different angles.
  4. 4Each organ system is examined systematically — brain, spine, heart, kidneys, limbs, face, and more. Measurements of the baby's head, abdomen, and thigh bone are recorded.
  5. 5The placenta, amniotic fluid, and umbilical cord are also evaluated.
  6. 6Once the examination is complete, the gel is wiped off, and you receive your report — typically on the same day at Anubhav Life Care.

The scan is completely painless and safe — ultrasound uses sound waves, not radiation. You may feel slight pressure from the transducer, but there is no discomfort. In some cases, you may be asked to walk around and return if the baby is in a position that makes certain views difficult.

Understanding Your Anomaly Scan Results

Normal Findings

A normal anomaly scan report means:

  • ✓ All organs are developing normally
  • ✓ Baby's growth is on track for gestational age
  • ✓ Placenta and amniotic fluid are normal
  • ✓ No structural abnormalities detected

The majority of anomaly scans show normal results.

Abnormal or Uncertain Findings

If an issue is detected, our team will:

  • ✓ Explain the findings clearly to you
  • ✓ May recommend a follow-up detailed fetal scan
  • ✓ Refer you to a specialist for further evaluation
  • ✓ Provide counselling and support

Some "soft markers" may need monitoring but are often harmless.
